OVERVIEW OF CADRE’S SUPPORT NETWORKS:
The Public and Community Health Support Network is one of 20 networks CADRE is developing to facilitate communication for nongovernmental organizations in Santa Clara County throughout the disaster cycle. These networks are broken up by different community services and functions to allow for more focused communication. The networks are designed to give organizations a way to quickly find and share information, collaborate, and communicate, before, during, and after disaster. Fill out the short Network Sign Up Form to ensure that you are included in updates about the networks you are interested in joining.
NETWORK UPDATES:
The Public and Community Health Support Network has not yet been developed. Any updates to the network will be noted here.
COMMUNICATION TOOLS:
1.) Web page – This web page and the forms, lists, and map within it will be updated regularly and used for continuous network coordination and information sharing during disasters.
2.) Live Chat – As the network develops, they will decide upon a live chat platform for communication during disaster. This live chat platform will provide major disaster announcements, urgent updates and requests, and welfare checks on organizations to quickly understand disaster impacts and immediate needs.
3.) Meetings – The network will decide upon a regular meeting schedule as it is developed. During disasters, meetings will be expanded at the network leadership’s discretion and by request.
4.) Back up methods of communication may include emails, phone and in person meetings, and utilization of Ham Radio and/or information centers in the case of technological challenges during an emergency. These backups are still being developed and decided upon.

Interactive Food Distribution Sites
This interactive map shows food resources in the County of Santa Clara, CA for COVID-19 shelter-in-place. The website allows you to look up the location of free meal food programs across Santa Clara County by entering your address to find locations close to you.
Interactive Map of Vaccine Resources
Santa Clara County, VTA has created an interactive map for the public to find a list of the current vaccination locations, along with the bus or light rail routes to take to each site. Transit trips are free as VTA is currently not collecting fares. When viewers click on any vaccination location on the map, they will be taken to the Google Maps transit option to find the most efficient transit route to get to the destination.
Meals on Wheels is for homebound individuals who are 60 years of age or older; who – due to illness or disability – are unable to purchase food or cook meals for themselves; and who do not have someone who can purchase food or cook meals for them on a consistent basis. To request this service, the form on the Sourcewise website can be completed or the phone number can be called.

Reach Out to Homebound – Sourcewise Ambassador Program
Human connection, fellowship, and friendship are good for the mind and soul. Volunteers bring a little joy and companionship to homebound seniors in our community.
Are you a natural nurturer who loves good conversation? The Sourcewise Ambassador Program is a perfect volunteer opportunity for you. Whether it’s a few minutes or an hour, a wellness call helps our elders feel connected during these uncertain times.
To become part of the Sourcewise Ambassador Program, you’ll be required to participate in a training session and make calls 1 or more hours per week. The commitment minimum is two months. Bilingual speakers are especially needed for Spanish, Vietnamese, Chinese and Russian homebound.

Call at least 2 days ahead of time to book a ride. There is no online booking.
They can also book vaccination appointments.
There is no eligibility criteria and the person doesn’t have to be low income, elderly or disabled. It’s the first 50 people who register each day.
They can take wheelchairs but there are limitations on other medical equipment. There can be up to 3 people in a booking.
